- Bus passes, when available.

Individualized housing-focused 90 day case management for adults and families in New Hanover, Pender, and Brunswick Counties.

Healthcare Assistance
Insured, uninsured, or underinsured? A Community Care of Lower Cape Fear Care Navigator is here to assist you and your family.
Services Provided:
- Apply for Medicaid, social security, or connecting with DSS and disability resources
- Link to primary care providers, dentists, behavioral health, vision providers, and other specialists
- Get medication education and financial assistance for essential medications
- Obtain referrals for care, durable medical equipment (DME), and supplies
- Address lifestyle changes such as smoking cessation and weight loss
- Improve health conditions such as diabetes and high blood pressure
