Who We Are
Cape Fear HealthNet's mission is to connect Cape Fear Region residents with low or no income to coordinated, quality and affordable healthcare resulting in a better, healthier life.
What We Do
Our role is to determine eligibility for our program, enroll and connect uninsured adults to a medical home and coordinate specialty referrals received from primary care partner clinics. Care Management is also available to our members and coordinated by our Referral Coordinator and partners, Community Care of the Lower Cape Fear.
